Arriving at your first appointment

If you are a new patient, please arrive 15 minutes prior to your appointment time to complete your medical/dental history form we require for your file.  Parents and/or guardians must accompany all patients under the age of 18 to their appointments.

Please Bring to First Visit

If your previous dentist has taken x-rays within the past 12 months you may request to have them forwarded to our office in advance of your appointment.  Generally, we do not request dental treatment records to be forwarded to us.

Please bring a list of any current prescription medications you are taking. Alert us if you require premedication before treatment.

Dental Insurance Information We Need:

            1.    Insurance Company Name

            2.    Group/Policy#

            3.    ID/Subscriber #

            4.    Percentages for Coverage 

            5.    Yearly Deductible Amounts

            6.    Limits or Exclusions

            7.    Which month is the beginning of your policy year

Any questions regarding your insurance coverage should be directed to your benefits coordinator at your workplace.  Your policy coverage is between you and your employer. 


Cancelling or No Show

We do require that you provide our office with 48 hours notice for canceling or rescheduling appointments.   We do understand that emergencies can arise, and for this reason, we will reschedule a missed appointment.  However, we are unable to reschedule patients that do not give us sufficient notice (either canceling on short notice or not arriving) for 3 scheduled appointments.

Arriving Late

When a patient arrives late for an appointment, it may or may not be possible to start or complete the treatment in the remaining reserved amount of time. To provide you with proper professional care, it may be necessary to reschedule your appointment or book an additional appointment to complete treatment.

Paying For Treatment

Our office policy is "payment is required at the time of treatment".  Please come prepared to pay your portion owing at the time of treatment.  We accept Cash, Checks, Most Dental Plans, Visa, Mastercard, American Express, Discover, and CareCredit Financing.  If you have dental insurance please present proof of coverage at your initial visit.  We will accept assignment of your insurance benefits.  However, we do require your co-payment and deductible be paid in full at the time of treatment.  Patients are fully responsible for payment of all fees associated with their dental treatment.
